IT Request Form

1.  Did you know that you can request items from IT through Okta? You can request new laptops, ipads, keyboards, monitors, and anything else IT related. 
   1. Log into you Okta account and click the ‘IT Request Form’ icon and fill it out. It’s that easy!

Triple check to make sure the email is your work email. Forms sent from personal emails are blocked.
